We’re quite happy to bill your Auto, Work, or Health Insurance. For all to go smoothly and make sure your insurance will pay your bills (rather than pass it on to you) it’s important that you provide us with the following information:
- Your Insurance card
- *A CURRENT prescription from a referring provider. This can be from an MD, DO, ARNP, PA-C, DC, or ND.
- If you’re seeing us for an auto or work injury, we’ll need your case manager’s name, phone, and fax number.
*(Your insurance plan may state that you don’t need a “referral”, which will be correct, but as secondary health-care providers, the state requires us to receive a prescription from your provider to show that we are not diagnosing any medical conditions which would be outside our scope of practice)
Which Insurance Companies do we bill?
We are providers for the following insurance carriers. Keep in mind that not all plans under these company names cover massage therapy. We strongly encourage you to pull out your insurance card, look on the back for the phone number, and call your insurance company to find out if they cover licensed massage therapy and for how many treatments. Although we check benefits, the insurance companies will often give better and more precise information to you, the subscriber. It’s best to know exactly what they provide upfront so that you don’t have any surprises later.

Keep in mind that your regular office visit co-pay is due at the time of treatment, and of course, you’ll be subject to whatever annual deductible your policy calls for.
No-Show/Cancelation Policy:
Please call us 48-hours ahead to cancel or change your appointment to avoid an office charge of $25. We are legally unable to bill your insurance company if you do not show up. All no-show/no-call appointments will be charged $65.
